Sometimes Minecraft doesn't correctly link together portals. Portals are suppose to have this relationship with each-other:
Overworld Portal X = Nether Portal X * 8
Overworld Portal Y = Nether Portal Y
Overworld Portal Z = Nether Portal Z * 8
Nether Portal X = Overworld Portal X / 8
Nether Portal Y = Overworld Portal Y
Nether Portal Z = Overworld Portal Z / 8
Sometimes, Minecraft won't perfectly connect portals together, creating discontinuous portals. This is because the location can be inside of a wall, and Minecraft will not place portals inside of a wall.
If you setup the portals perfectly, like in this video, they will always work one-to-one.
